College of Western Idaho (CWI) is committed to fostering supportive and safe community for students. A range of resources are available on campus to address student care and well-being, academic integrity, misconduct, and to ensure a respectful community.

Care & Concern
Forms may be submitted anonymously and be used by anyone, regardless of their interaction with CWI.

Responses to a report can happen within two (2) business days, sooner if communicated.

While confidentiality will be maintained through the reporting process, reports may be subject to disclosure through FERPA, FOIA, subpoenas, or other means.

Student of Concern Referral Form: Connect a student with support who may be experiencing academic difficulties, personal or family issues, or is in need of advising, coaching, or basic needs.

Pregnancy & Parenting Form: Connect students with support and resources who are pregnant, expecting a child, have recently given birth or had a child, expecting to adopt a child, lost a pregnancy, is undergoing fertility treatments or other medical procedures related to pregnancy, or is experiencing discrimination or unfair treatment due to an unexpected pregnancy or their status as a parent.

Notification of Student Death: Report a student’s passing to the College.

Student Conduct

Student Misconduct Form: Report student behavior that violates the Student Code of Conduct.

Student Conduct Appeal Form: Appeal a student conduct decision.

Sanction Form: Verify completion of sanctions as part of the College’s student conduct process.

Registered Sex Offender Self-Report Form: Report your sex offender status as a student to ensure you are not violating terms and conditions of your parole or probation. This also allows the College to ensure minors are not in classes or co-curricular opportunities in which you are participating.

General Complaints: Report concerns/complaints not resolved through direct communication.
Respectful Community

Respectful Community & Title IX (Sexual Harassment) Report: Report incidents of sexual misconduct, sexual harassment, dating or domestic violence, gender-based harassment or bullying, and/or stalking in which the affected person is a student. This form can also be used to report incidents of harassment, bias, or discrimination on the basis of race, color, national origin, ancestry, religion, marital status, age, physical or mental disability, or military or veteran status.
