This form is used to exercise your right of appeal to academic policies, requirements, student account charges, etc. Please carefully read and follow the directions in the information section of this form. For further information please refer to the Student Handbook - Academic Record Appeal Procedure .
This is NOT the appropriate form for appeals related to Financial Aid and Satisfactory Academic Progress.
Before you begin the appeal process, it is important to remember, that you, the student, are held responsible for deadlines, policies and financial obligations. Sometimes, extenuating circumstances allow for exceptions. If you submit an Academic Record Appeal requesting an exception, it is your responsibility to provide documentation and justification for consideration by the Academic Record Appeals Board. The board will review an Academic Record Appeal one time. A re-appeal of the same request will not be accepted by the Academic Record Appeals Board.
For your Academic Record Appeal to be considered the following MUST be provided:
1) Completed Academic Record Appeal Request Form (This form)
2) A one-page letter of explanation (May be typed and attached to this form)
3) Appropriate documentation to support your request (Required)
You will be notified by email regarding the outcome of your Academic Record Appeal Request - approved or denied. Make sure your contact information is current with the College. The Academic Record Appeals Board reviews Academic Record Appeal Requests weekly. You should expect to receive notification within 10 business days of the submission of your Academic Record Appeal Request. Failure to
provide supporting documentation may result in a denial of your appeal.
